Thank you for considering Christian Heritage Academy Upper School!  Student Application forms may be completed and submitted online using the links below.  Please be sure to read the instructions on each individual form carefully. 

Administrator Recommendation Forms must be completed by one of your child's current or former school administrators. Pastor Reference Forms must be completed by the current pastor of your family's home church. You may email the link to this page to your pastor or school administrator so they can complete and submit their form online, or you may provide them with the printed form. 

Upper School Enrollment Check List
Upper School Enrollment Applications must include:

____ Completed CHA Upper School Student Information Form 

____ $125 non-refundable application fee

____ Completed Administrator Recommendation Form (required with all CHA applications and may be mailed, emailed or faxed to CHA separately from application. Please feel free to provide a link to this page to the administrator so that he/she may complete and submit the form online.)

____ Completed Pastor Reference Form (required with all CHA applications and may be mailed, emailed or faxed to CHA separately from application. Please feel free to provide a link to this page to your pastor so that he/she may complete and submit the form online.)

____ Parent Interview (to be scheduled through Admissions after receipt of Application materials)

____ Referral Incentive form (if applicable)

Fee Payment- Once Applicaton Form is complete,please submit your application fee(s) payable to Christian Heritage Academy. Checks can be delivered in person or mailed to:
