Thank you for considering Christian Heritage Academy Upper School! Student Application forms may be completed and submitted online using the links below. Please be sure to read the instructions on each individual form carefully. Administrator Recommendation Forms must be completed by one of your child's current or former school administrators. Pastor Reference Forms must be completed by the current pastor of your family's home church. You may email the link to this page to your pastor or school administrator so they can complete and submit their form online, or you may provide them with the printed form. Upper School Enrollment Check List ____ Completed Administrator Recommendation Form (required with all CHA applications and may be mailed, emailed or faxed to CHA separately from application.
